Yes

5 ) Guild/Server History, why you left previous guild,server, etc.
Core Gaming: was there 1 day and promptly realized that was not a good environment for me.
Carnage: Guild did not have the tools to properly function (missing classes for buffs etc) and leadership was not well organized.

6 ) If currently guilded why do you want to leave it. What other guilds have you applied to.
I have applied to Midnight and a few off server guilds.

7 ) Do you have any prior raiding experience pre expansion and/or post expansion? If yes explain, be specific as possible.

I raided as an arcane mage for the end of vanilla (when I started playing). I did MC/ZG back then.
I was a Feral Druid for T4/5 Content during its release, and a Moonkin for T5/6/Sunwell (up to Brutallis) (guild had me respec to moonkin to buff warlock dps... lol shadowbolt...)

Wotlk:
Switched mains after Naxx (Moonkin to Rogue) and left the game for a brief period during ulduar.

Life... outside... WoW? I don't believe it.
I enjoy movies with the wife, and building computers, and playing other games.

13) What is your proudest achievement?
Well it wasn't an achievement back then, but popping that crucial tranquility during phase 3 Illidan and getting the kill sure as hell felt good. (Was moonkin then)

14) What do you like most about your class/spec?

I love the speed of combat, and I love the control I have with my gear. I am very much a min maxer when it comes to gear and pre plan my gear using chardev.org. I love mutilate for it's simplicity allowing me to focus on other things outside of my role and further understanding the encounter better.

I know my expertise is under the soft cap and I'm missing a raiding profession. I'm working on my 4 piece (4 badges away as of this post and the mining is to help me fund buying the Ikfarus Sack of Wonder. As soon as I have that (I'm building up another gathering profession on my alt atm), I'll be switching to JC or Enchanting for my 2nd profession.
